        吸吮线虫是寄生于哺乳类和鸟类泪管、瞬膜或结膜囊内的一类线虫。大部分为动物寄生虫，只有结膜吮吸线虫（Thelazia callipaeda Railliet &Henry,1910）有人体寄生的报道。感染本虫的犬和猫等为传染源。由蝇类舔吸带虫动物的眼部后，再舔吸其他动物或人而构成传播。犬感染结膜吮吸线虫一般无明显表现，重者流泪，结膜潮红肿胀，翻开眼睑在眼角处可见到白色线状虫体。人的一般在蝇类繁殖季节过后发病，表现结膜充血，羞明、流泪。犬和猫都有结膜吸吮线虫感染，作为伴侣动物与人关系密切，为防止伴侣动物带虫传染人，应及时诊治犬猫结膜吸吮线虫病。治疗可用0.5%敌百虫溶液点眼或1%左旋咪唑溶液点眼，然后用镊子将虫体取出。

        结膜吸吮线虫又称华裔吸吮线虫，是一种寄生在狗、猫等动物眼部的线虫，亦可寄生于人的眼部，引起结膜吸吮线虫病。在我国重庆首次发现本虫寄生于狗的结膜囊内。人眼结膜吸吮线虫病例最早则发现于北京和福建。此后，国内外陆续有本虫寄生人眼的报道。本病多发生于亚洲地区，故称东方眼虫病。寄生于人眼的吸吮线虫除本虫之外，还有加利福尼亚吸吮线虫。它主要见于美国的加利福尼亚州，亦曾见于西雅图。成虫细长，在眼结膜囊内寄居时为淡红色，离开人体后，呈乳白色、半透明。头端钝圆，具圆形的角质口囊，无唇。口囊外周具两圈乳突。体表具有微细横纹，横纹边缘锐利呈锯齿形。雄虫：长4.5～15.0mm，宽0.25～0.75mm，尾端弯曲，交合刺两根，长短不一，形状各异。雌虫：长6.2～20.0mm，宽0.3～0.85mm，生殖器官双管型，子宫内虫充满虫卵。虫卵椭圆形，壳薄，大小为54～60μm×34～37μm，内含幼虫。卵在产出之前，卵壳已演变成包被幼虫的鞘膜。

        该虫在我国的传播媒介为冈田绕眼果蝇(Amiota okadai)，在意大利等欧洲国家为Phortica veriegata。还发现我国的另一种大饶眼果蝇A. magna也可作为结膜吸吮线虫的中间宿主和传播媒介，纠正了以往把家蝇作为本病传播媒介的误解。

6 症状体征

        成虫在人体多侵犯一侧眼，少数病例可双眼感染。主要在上下睑穹隆内，也寄生于泪腺、结膜下及皮脂腺管内。寄居虫数可多达21条。患者眼部病变可因虫体体表锐利的横纹摩擦、头端口囊吸附作用以及排泄分泌物的 *** 作用而引起，导致炎症反应或肉芽肿形成。轻者可无明显症状，也可有眼部异物感、痒感、流泪、畏光、分泌物增多等临床表现。婴幼儿有不敢睁眼、似手抓眼的表现，常因家长发现患儿结膜有白色小线状虫爬行而就诊。取出虫后症状消失。严重者可伴有结膜充血、小溃疡面形成或角膜混浊及眼睑外翻等。若虫体寄生在前房，可出现眼部丝状阴影移动感，睫状体充血、房水混浊、眼压增高、瞳孔，甚至视力下降。泪小管受损，可导致泪点外翻。

        成虫主要在终宿主狗、猫等动物的结膜囊及泪管内寄生，偶可寄生在人的眼部。雌虫在眼眶内排出具有鞘膜的初产蚴，当其中间宿主蝇等在宿主眼部舔食时，初产蚴随眼分泌物进入蝇的消化道，穿过中肠侵入血腔，经两次蜕皮发育为感染期幼虫并进入蝇的头部。当该蝇在舐食其他宿主眼部时，感染期幼虫自蝇喙逸出，进入终宿主眼部，在15～20天内幼虫再经2次蜕皮发育为成虫。从感染期幼虫进入终宿主至发育为开始产卵的成虫所需时间大约为50天左右。成虫寿命可达2年以上。

9 诊断检查

        本病根据自患眼取出虫体，镜下鉴定即可确诊。

10 治疗方案

        治疗方法简便，可用1％地卡因、4％可卡因或2％普鲁卡因滴眼，虫体受 *** 从眼角爬出时用镊子取出，或消毒棉签取出即可。然而用3％硼酸水冲洗结膜囊，并点滴抗生素。若虫体寄生在前房可行角膜缘切开取虫，术后作抗炎等处理。虫体较多者，常须多次治疗。

11 预后及预防

        预防本病的关键在于注意个人眼部卫生，特别是幼儿，应保持眼部清洁，加强犬、猫等动物管理，以及灭蝇、防蝇等措施。

12 流行病学

        本虫主要分布在亚洲。印度、缅甸、菲律宾、泰国、日本、朝鲜及俄罗斯的远东地区均有病例报告。我国的病例报告始于1917年，为世界最早发现。现已知我国有25个省、市、自治区有人体感染的病例报道，其中以江苏、湖北、安徽、河南、山东等地的病例较多。近年已证实冈田绕眼果蝇（Amiota okadai）是我国结膜吸吮线虫的中间宿主，是本病的传播媒介。感染季节以夏秋季为主，与蝇类的季节消长相吻合。感染者最小3个月，最大者88岁，但以婴幼儿为主。本病在农村多于城市。传染源主要为家犬，其次是猫、兔等动物。根据1982～1991年的调查，安徽淮北重流行地区的某些村镇家犬的感染率高达76.7%，每犬的感染虫数可在30～60条。保虫宿主家犬的普遍存在，媒介中间宿主果蝇的广泛分布，再加上幼童不洁的眼部卫生，是结膜吸吮线虫病流行的主要因素。

        本虫主要分布于亚洲。在我国，迄今已报告人体结膜吸吮线虫病331例，分布于除青海、西藏、宁夏、甘肃、海南及台湾外的25个省（市、区），其中以山东、湖北、江苏、河南、安徽、云南及河北报道的病例较多。
